Transcript and Recommendation Requirements

Thank you for submitting your Summer Research Internship Program On-line Application.  All subsequent parts of your application, official transcripts and completed reference forms, will need to be received by February 15, 2013.

All transcripts should be mailed to:

B. Submit an official copy of your current college transcript, and any from previously attended colleges or universities. Please have transcripts sent to the address above.

C. Please have three faculty members familiar with your research or academic background complete the SRIP Recommendation Form for you.  The application allows you to enter the names of a maximum of four referrers. We must receive a minimum of two letters of recommendation to review your application.

Instructions for Applicant:
1) Open the form in the desired format.  Note: if you are using the PDF form your recommender will need Abobe Acrobat or another PDF editor. Adobe Reader cannot save PDF changes.

2) Add your personal information in the first two fields, select "SAVE AS", and RENAME the document TO PREPARE IT for your referrer and the UVA SRIP Program. Please change the name of the document, combining your last name, and the last name of your referrer (ex. Jones_Smith.doc) and forward the form to your recommender for completion and submission.

Alternately, you may supply your referrer with a link to this page and they can retrieve the form that best suits their needs.
