Ph.D. Degree and BIMS Program Declaration Guidelines
|1) The “Ph.D. Degree and BIMS Program Declaration Form” must be
used to invoke all changes in “Ph.D. program” and “graduate group”
2) Changes in a student’s Ph.D. degree-granting program may be submitted at any time using the form.
Note: If you select a degree granting program other than those listed on the form, please have your graduate adviser submit a letter which includes all of the following information: amount and dates of your financial support, name of degree plan, and brief explanation as to why you are selecting this particular major in which to pursue your Ph.D. Funding Policy Guidelines can be found here
3) Forms should be submitted to the Graduate Programs Office (GPO)in Jordan Hall without the Assistant Dean’s signature but with all other signatures.
4) BIMS Graduate Group changes will need to be approved by the Assistant Dean of Graduate Research and Training by April 25th of each year in order for GPO to accommodate, pre-set administrative deadlines within the University (such as Budget, Registrar, Bursar, GSAS Office deadlines). No changes in BIMS group affiliation will be enabled between April 25th and August 15th.
5) GPO staff will coordinate the Ph.D. program assignment with GSAS, will enter changes in the GPO databases and maintain a permanent copy of the form. BIMS Administrators will be sent a copy of the form for the student's file after the form has been processed.
