How do I Obtain Written Reports?

UVA Physicians:  Written reports can be obtained from E-Care (Centricity). If you need help getting into E-Care, please call the Help Line at (434)924-5334. 

Non-UVA Physicians:  Call the Medical Records at (434) 924-5136 for assistance. You will be asked to fax a written request prior to a report being released to your office.

Outside Physicians: If you wish for your reports to come to your office via fax versus through the mail, please call Sharon Kuhns at (434)982-3435 or e-mail her at: SMK3S@virginia.edu
