Ward Responsibilities

I.      Old Patients

A.      - On-service Notes: No more than 1/2 page

- Brief hx summary

- Pertinent PE

- Plans

B.        Daily progress reports on 7:30 AM rounds:

- Name and Dx

- Present Status: Vita Sign, Weight, I & O,

Physical Finding, Therapy at Present

- Results Back

- Plans (X-rays, lab, rx)

C.        Daily progress notes (Talk to Intern First!)

- Present Status (See Daily Progress Report)

- Results Back

- Plans

D.        Consults, Orders, etc Must Be Countersigned.    Performing procedures is not a primary purpose of this clerkship. You should become familiar with procedures through observation and assistance.

II.          New Patients

A.        Presentations on Morning Rounds when applicable.

-  Time Limit 3 minutes

-  Brief, Summarized HPI

-  Pertinent PMH, FH, SH, and Physical Findings

- Plans

B.        Write-Ups:

-  No more than 2 detailed write-ups per on-call night.

-  Concise and Pertinent HPI

-  Complete ROS, PMH, FH, SH, and Physical Exam

-  At the end, give a formulation, differential dx & discussion of patient's case in terms of what you read; reading is primarily limited to textbooks, review articles may be used.  List references after work-up.

C.    U/A, Diff, PPD, Growth Chart on the chart by next a.m.; also, have all pertinent lab data on chart by next a.m.

III.         Night Call

3 PM to 8 AM .  Students may use the on-call room on the 7th floor.  Ask the Resident for key.

-  Present patient following a.m. on rounds
