To initiate a referral:
Physicians may refer patients directly by calling our toll-free number
above and speak to our referral coordinator, who will answer your
questions, collect the necessary information required for the referral
process, and schedule an appointment with our neurosurgeons. Please
have the following information available at the time of your call:
· Patient name
· Diagnosis
· Details of diagnosis (physical exam, biopsy, x-rays,
etc.)
· Specific information about any treatment the patient has
received
· Patient's telephone number
· Patient's insurance information, including any necessary
HMO authorization
Our physicians welcome your calls. Together, we can review your
patient's medical history, discuss your patient's needs and determine
the best course of action.
After a Referral is Made
We will call your patient for pre-registration and discuss what he or she can expect at the first visit. An orientation packet will be sent to your patient prior to the appointment with pertinent information about their visit, and logistical information, such as location, directions, and parking. We will contact you and provide a copy of any written notes on your patient's procedure. Patients will be directed back to the referring physician for follow-up care. Depending upon the condition being treated, the effects of treatment may occur over a period of time. The faculty and staff of the center will work with you in monitoring the effectiveness of the therapy and encourage your participation in every phase of your patient's care.
