another physician, please be sure they fax your pertinent medical

crucial in you receiving the best care from us and incurring the least
out-of-pocket expenses.
- If you are referring yourself to our
clinic, please fax (to the number above) or bring with you all medical
records you may have from previous doctors or hospitals so we can
provide the best medical care possible.
- Please arrive at least 10-15 minutes
before your appointment to allow time for
check-in, registration, and any paperwork.
- Please bring all medications with you to
your appointment or have a list of your medications.
