2.  Leave the following information:

•   Your full name and date of birth
•   The name of each medication, dose, and frequency
•   Whether you wish a one or three month prescription
•   The name of your clinic physician
•   The name, location, and phone number of your pharmacy
•   A contact phone number where we can reach you
•   For mail-order pharmacies, your identification number
