Standard Appointment

Call 434-924-2959 to schedule an appointment. Physician preferences will be accommodated. If you have no physician preference, your patient will be scheduled in the first available appointment. Our goal is to accommodate appointments within two weeks. 

Provide the following information:

• Patient name, address, phone number, insurance company
• Copy of the pre-authorization of referral from the patient's insurance company
• Copies of the most current lab tests, radiology reports and pertinent clinical notes

An appointment notice will be mailed to the patient prior to the appointment date.

Results of the patient's visit will be mailed to the referring physician within one week.
