About the Practice

Crossroads Family Practice provides continuing and complete health care to individuals and families. We hope that our medical practice can be your "home base" for medical care.

Crossroads Family Practice has been established by the Department of Family Medicine at the University of Virginia Health System. We provide General medical care including obstetrics and minor surgery. We care for patients of all ages. When hospital care is in order, we admit our patients to University Hospital where they will be cared for by the Department of Family Medicine and by any specialists that may be needed.

Where Are We?

We are located in North Garden, Virginia, 11 miles south of Charlottesville at the Crossroads Corner Shops (U.S. 29 and Route 692). We provide Medical Care to North Garden and surrounding areas of Albemarle, Nelson, and Fluvanna Counties. However, we accept individuals and families wherever they may reside.

Office Appointments

We see patients in the clinic by appointment which can be made by calling (434) 243-4660.

Monday, Wednesday, Friday

9:00 am -- 5:00 PM

Tuesday and Thursday

9:00 am -- 7:00 PM

We will see patients without appointments as time is available. Emergency cases will be given priority.

If you or a family member needs medical care during the night or on weekends, call (434) 924-0000. This is the main switchboard at the UVa Medical Center. Ask for the family medicine doctor on call. The doctor, who is a member of the Department of Family Medicine, will return your call as soon as possible. He or she can give you medical advice over the phone and, if needed, will meet you in the University Hospital Emergency Room. It is important to call before coming to the Emergency Room. Your stay in the Emergency Room can often be shortened if the doctor knows that you are coming.

We Are Prepared To...

  • Care for new or old illnesses you may have
  • Treat cuts, sprains, and other injuries
  • Care for you during your pregnancy and deliver your baby
  • Provide counseling for families
  • Take care of your baby as well as your older children
  • Provide care to older persons
  • Provide birth control information and services
  • Provide preventative care and health maintenance services
  • Provide first-line and continuing medical care for you and your family. If needed, we can arrange for you to see medical specialists.
  • Collect blood or other specimens for laboratory testing
  • Schedule any needed X-ray studies for laboratory testing

Medication Requests

If you need a prescription refill, please call the office between 9:30 am and 4:00 PM, Monday through Friday. Please have the medication name, strength, supply needed (30 day or 90 day), pharmacy and patient name readily available. This information will allow us to have your chart available for the physician to respond to your request. Requests after 4 PM may not be reviewed until the next day. Please allow two business days for us to call in new prescriptions. Please call your refill requests in as early as possible to avoid delays.

Charges and Insurance

The Crossroads Family Practice operates on a fee-for-service basis. To keep paperwork simple and our charges as low as possible, we ask that you pay when you come for an appointment.

If you are covered by insurance, we will file all reports of your medical care with your insurance agencies.


If you or a family member are having a stroke, heart attack, or other serious or possibly life-threatening illness, call 911 or your local rescue squad. If time allows, please call the Crossroads Family Practice or the family medicine doctor on call at UVa: (434) 924-0000.

If the emergency is not life-threatening, please call Crossroads Family Practice instead of going straight to the Emergency Room. You can avoid unneeded and costly trips to the Emergency Room by calling us first.

If for any reason you are taken to the Emergency Room, be sure to tell the E.R. receptionist that you are a patient of the Crossroads Family Practice.
