Required Logs

Family Medicine Clerkship students are required to keep two logs during their clerkship.
You will receive a copy of the Preceptor log in the student binder during orientation.

The preceptor log is a blank schedule where you indicate how many half-days you worked with each physician.

 Oasis Requirement Checklist Log

This is a list which encompasses the essence of what you must experience during your four week rotation (including diagnoses, presenting complaints and preventive health).  For each of these cases, you must seek out involvement in the history, diagnosis, physical examination, and management of these patient encounters and document your level of participation. After you have completed the required number of cases, you should log in your patient encounters in Oasis.
