Appointments & Referrals

photo physicians consultingThe faculty and staff of the Department of Dermatology provide patient care services through outpatient clinic appointments, inpatient consultation, and the admission of patients to the University Medical Center.

The University of Virginia considers Referring Physicians to be our most important partners in providing comprehensive care to patients. The information below is designed to help you and your staff refer patients quickly and efficiently.

For Patients

Outpatient Clinic Appointments

Please call 434-924-5115 to schedule an appointment.

The Dermatology Clinic is open:
Monday to Friday, 8:00AM - 5:00 PM
3rd Floor, Primary Care Center
UVA Medical Center

Physician preferences will be accommodated. If you have no preference, you will be offered the first available appointment. Our goal is to accommodate appointments within two weeks.

Please bring this information with you to your appointment:

  1. Your health insurance card or information
  2. Copy of pre-authorization from your insurance company (if required)
  3. All medications your are taking (in a bag), or a current list with name, dose/strength/amount, and times that you take the medications

An appointment notice will be mailed to you before your appointment date.

Hospital Admission

If you are coming to be admitted to the hospital, please bring with you:

  1. Your health insurance card or information
  2. Copy of pre-authorization from your insurance company (if required)
  3. All medications you are taking (in a bag), or a current list with name, dose/strength/amount, and times that you take the medications
  4. A copy of your Advance Directive or Living Will, if you have one

For Referring Physicians

Outpatient Referral

Please call 434-924-5115 to schedule an appointment in our general or subspecialty clinics. Patients in need of an urgent appointment will be seen within 24 hours.

You may fax a referral or patient medical records to 434-924-5936.

Please provide the following information:

  1. Patient name, address, phone number, insurance company
  2. Copy of the pre-authorization for referral from the patient's insurance company
  3. Copies of the most current lab tests, radiology reports and pertinent clinical notes

An appointment notice will be mailed to the patient prior to the appointment date.

Results of the patient's visit will be mailed to the referring physician within one week.

Hospital Admission

If you do not have a physician preference, call the operator at 434-924-0000 and ask to be connected to the on-call physician for Dermatology.

The attending physician or fellow will facilitate patient transfer through the Bed Coordination Center.

When a bed is available, the referring physician will be notified.

If a waiting list exists, the Bed Coordination Center will communicate daily with the referring physician to update transfer status.

Please provide the following information:

  1. Patient name, address, home phone, work phone
  2. Date of Birth
  3. Insurance provider and policy number
  4. Name of physician requesting transfer
  5. Name of hospital and phone number of unit from which patient is being transferred
